To order a Plaque, selected a Customized or Standard Plaque below and fill in the Date of Service (including Start and End Date), and the Rank and Serial Number. If selecting a Standard Plaque select a ship from the drop down menu. Then click Add to Cart. You will be forwarded to a paypal cart where you can purchase the plaques using your credit or debit card.
